BibIndex: author index include/exclude first names

• New config variable CFG_BIBINDEX_AUTHOR_WORD_INDEX_EXCLUDE_FIRST_NAMES is introduced to govern whether to include or exclude first names when creating author word index. The default is to include first names, as it used to be traditionally. (closes #219)
