refextract: use appropriate marc tags

• Places author and affiliation information into $100/$700, into subfields $a and $u respectively.

• Cleans up comments and includes author-extraction related functions for the processing of xml output.

• Updates refextract_tests with the correct call to the 'display xml lines' function (for references as opposed to authors/affiliations)
