inlay tr templates into main
name objects in scopes, update helpers and event handlers
use ReactiveVar instead of Session var (different values for different
studies)
update studyDesign.questionnairesIds and studyDesign.recordPhysicalData
according to child visit entries
hide recordPhysicalData if not scheduled nowhere yet
