feat: Original code to measure muscle degeneration

Original MATLAB code to generate a 2D CT slice oriented in a given plane
to contour the rotator cuff muscles. From the contours, muscle
degeneration parameters are computed: muscle atrophy, fat infiltration,
and osteochondroma.
