Add "and" support to "ref"

Summary: Fixes T8038. Allow PhabricatorCustomFieldMonogramParser to handle "and". This means that Ref Tx, Ty and Tz will correctly return array('monograms' => array('Tx', 'Ty', 'Tz').

Test Plan: Added unit tests.
