feat: Remove unnecessary steps in segmentation

Removes unnecessary code for the segmentation of the different muscle
regions (median filtering of the orginial image, islands removal and
morphological operations to smooth the contours)
These operations had no impact on the estimation of fat infiltration or
osteochondroma. Their removal slightly lowers the estimation of muscle
atrophy, but avoids scientifically invalid steps for the segmentation.
